After reading story after story over the past week about the Target breach, I can't help but feel like part of the story is missing. By now everyone is familiar with the who, what, where and when of the situation – but we're consistently missing out on the WHY and, more importantly, the HOW of this breach.

It's understandable, at least partly. The why and the how are the two more difficult questions in journalism, and Target naturally isn't opening up on the how front while the investigation is underway. So far we've only received the usual details of malware and a "sophisticated" attack.
